Anesthesia Patient Safety Foundation (APSF) - Representing one specialty's focus on improving safety, the APSF web site records the history of their effort, as well as updating current activities. Search their eighteen-year newsletter archive, or visit relevant links, grant program details, clinical information and safety videos.
Quality and Patient Safety / Institute of Medicine (IOM) of the National Academies - Anyone interested in patient safety should be familiar with the 1999 report To Err Is Human. The IOM site has many other reports and workshop presentations dealing with health care safety and quality.
Healthcare 411 Audio Series - The Agency for Healthcare Research and Quality (AHRQ) provides free podcasts and transcripts of stories related to patient safety.
Hospital Compare / Department of Health and Human Services - This tool provides you with information on how well hospitals care for all their adult patients with certain medical conditions, which will enable you to compare the quality of care hospitals provide.
Institute for Healthcare Improvement (IHI) - The Institute for Healthcare Improvement is a non-for-profit organization leading the improvement of health care throughout the world. IHI was founded in 1991 and is based in Cambridge, Massachusetts. The IHI has implemented the Protecting 5 Million Lives from Harm Campaign.
Institute for Safe Medication Practices (ISMP) - The ISMP provides guidance, tools, and announcements to raise awareness of incidents and issues in medication error and administration, primarily patient safety concerns. The ISMP Medication Safety Alert hospital and ambulatory pharmacy assessment tools serve as valuable content for point-to-point dissemination efforts to clinicians.
Leapfrog Group for Patient Safety - The Leapfrog Group is an organization committed to improvements in safety, quality and affordability of healthcare for Americans. The web site contains the results of the Leapfrog Hospital Quality and Safety Survey, which is searchable by hospital name, state, city, or zip code. You can also create a chart to compare the results of multiple hospitals.
Medical Errors & Patient Safety / Agency for Healthcare Research and Quality (AHRQ) - AHRQ is the primary government-finding agency for patient safety research. This site includes a patient safety page with tips for patients, clinicians and researchers, the Quality Indicators project, a review of the evidence base for patient safety, and the new online journal Web M&M that provides a peer-reviewed collection of "lessons learned" through discussions of actual incidents.
Michigan Health and Safety Coalition - This organization is a collaborative quality improvmeentn effort focused on improving patient safety in Michigan. Annual reports on specific Michigan hospitals are included.
National Patient Safety Foundation (NPSF) - The NPSF provides broad coverage of topics needed to understand patient safety. A comprehensive, multidisciplinary bibliography serves as the online catalog of the Patient Safety Information Resource Center. There is also full-text access to grey literature, a safety glossary, information on its membership program, and a compendium of primary research efforts.
Patient Safety / Joint Commission on Accreditation of Healthcare Organizations (JCAHO) - The Joint Commission's focus on safety is reflected in its standards and goals. Notable areas on this site include "Facts About Patient Safety," a five-year, sentinel event alert archive and a sentinel event glossary.
Quality and Patient Safety / American Hospital Association (AHA) - This section of the AHA site links to reports, surveys, and other field-tests tools for studying quality and safety, and includes an award program for hospitals that have done exceptional work in enabling and sustaining a culture of safety. Also included are the failure mode and effects analysis and disclosure documents for the American Society of Healthcare Risk Managers (ASHRM), a part of the AHA.
