EAP- TYPES OF REFERRALS
Self Referral
An employee or member of employee's family is seeking counseling on their own. There is no Manager/Supervisor/HR involvement.
Informal
Manager/Supervisor/HR encouraged contacting the Employee Assistance Program (EAP) for any help employee might need in addressing any problem(s) that may be affecting their recent job performance.
Formal
Manager/Supervisor/HR requires that employee be seen by EAP regarding stated problems/concerns. No disciplinary action taken at this point.
Disciplinary Action
Manager/Supervisor/HR requires that employee be seen by EAP regarding the stated problems/concerns as part of a disciplinary process. Future disciplinary actions may occur
